I became a doctor because I liked certainty. Fifteen years of practice cured me of that, and made me useful.
In clinic I kept meeting the same patient: smart, motivated, exhausted by advice. They had read that eggs were poison and then that eggs were a superfood. They owned three wearables and slept worse than ever. What they needed was not more information. It was a filter.
So I built one. The Signal started as a Friday-night email to forty patients. It is now a weekly briefing read by thousands of physicians, founders, and curious people who want the truth at the speed of one coffee. I still write every word myself, and I still change my mind in public when the evidence changes first.
